I don't know what forms are available on LPA.
When I have done it in the past, the new contract will state it is an amendment to the lease dated MM/DD/YYYY.

Identify what portion of the lease is being amended (occupants, and who are tenants).
Then have all persons sign it.

Also consider how the security deposit is to be handled. I would recommend you have the tenants deal with that among themselves. Have the lease sever the ability for the person leaving to collect any return in the security deposit.

Remember, if you are letting the only person who is responsible off the hook, your chance of getting stiffed financially will increase.

Thank you sir. The lady I am letting off the lease is on ssi and is dying from lung cancer. I see no reason to put her through this stress and for some reason it is very stressful to her. She has nothing to give financially and no assetts of any kind so I see no reason to disrupt her peace.
